7th Grade Health Requirements

The State of Florida laws requires that students entering 7th grade must have both of the following items turned in to our Front Office BEFORE starting school.  

A School Physical Examination (by doctor/healthcare provider) on Form 3040 is required and must be dated on or after August 11th of the previous year.

Part 1 of the Physical Form - Must be completed and signed by the parent/guardian and Part 2 - Must be completed and signed by the doctor/healthcare provider.

Immunization Form DH680 with the following (handwritten documents are not accepted):

    • Tetanus/Diphtheria/Pertussis (Tdap)
    • Varicella series (chicken pox) vaccine OR documentation of history of Varicella Disease
    • The Hepatitis B Vaccine series
    • MMR Vaccine series (measles, mumps and rubella)
    • Polio vaccine series
    • Diphtheria-tetanus-pertussis vaccine series

Note: If an incomplete Form DH680 is presented to the school, it is invalid and must be returned to the parent/guardian for physicians’ correction.

All students trying out for sports must submit a completed athletic packet online at Aktivate.com before tryouts begin.  The EL2 form may be completed on any date, all other paperwork must be completed on or after July 1 of the new school year.  Paper copies will NOT be accepted.
